Functional antibodies should only be requested if there is a clinical suspicion of antibody deficiency (primary or secondary). The recommended first line Immunology tests are:-
• Immunoglobulin levels (IgG, IgA and IgM)
• Electrophoresis
The second line testing, which includes functional antibodies, should only be requested if:-
• There is a strong clinical history of recurrent or persistent infection
• At least 2 Immunoglobulin isotypes are lower than reference limits
• To assess immune response to vaccines (pre- and post-vaccine)
This assay is used to monitor IgG responses to the Prevenar and Pneumovax vaccines, in patients with suspected immunodeficiency. Samples should ideally be tested pre- and post- vaccination. The serotypes available are 1, 3, 4, 5, 6b, 7F, 9V, 14, 18c, 19A, 19F and 23F which are in both the Pneumovax and Prevenar vaccines.
