Test Category: Specialist Biochemistry

Investigation Name:Red Cell Cholinesterase

Accreditation:UKAS ISO15189:2022 accredited test: On scope

Intro:

Poisoning due to organophosphorus and carbamate insecticides is uncommon in the UK though fatalities still occur. Organophosphorus and carbamate insecticides covalently inhibit neural, erythrocyte and plasma cholinesterase to varying extents and with different time courses.
Acetylcholinesterase is present in human erythrocytes as well as in the target synapses and the activity of acetylcholinesterase in red cells is assumed to mirror that in the target organs (liver, pancreas, heart, white matter of the brain).

Pathology Laboratory:Manuals Laboratory

Requestable Seperately?Yes

Minimum Sample Volume:2mls plasma EDTA preservative.

Expected Turnaround Time:1 working day

Sample Type:

EDTA whole blood is required.
Cholinesterase is stable in whole blood without loss of activity for 7 days at 2-8 0C and for <24hrs at 15-300C.
Samples should be brought to room temperature and mixed before analysis.
Red cell only samples, i.e., samples which have been undergone red cell washing and separation from plasma, are stable at -20°C for one month, but consideration of a possible decrease in activity of 16-32% is essential during result interpretation.

Complex Reference Range:≥2777 U/L

Collection Conditions / Other Information:Classed as an urgent test. Please contact the laboratory prior to sending sample.
Same day transport – ideally within 6 hours of collection.
Notification on the expectation time of samples is ideally required.
Send without ice / cool pack unless same day transport is not possible, in such scenarios it is advised to store at +2-8 C° and transported at +2-8 °C.
If sample cannot be sent on the same day, please store at +2-8 °C and inform laboratory when sample is being sent.
